Acute appendicitis is one of the most common surgical emergencies encountered in children. In our institute, we carried out a study to identify if there is a direct pathological correlation between SARS CoV-2 virus and complicated acute appendicitis. A retrospective study was carried out comparing data of patients with acute appendicitis Pre-COVID and During-COVID. Analysis revealed a higher incidence of complications such as perforation, gangrene and abscess formation during COVID pandemic and the duration of symptoms between both groups was statistically insignificant.4 patients had tested positive for anti-COVID antibodies. This suggests Multi-system inflammatory syndrome in Children [MIS-C] related to SARS-CoV-2 Virus possibly contributed to complications. This is explained by a)the appendix being one of the targets of the virus due to expression of ACE2 receptors on glandular cells, b)children with MIS-C more often test positive for antibody to SARS-CoV-2 than nasopharyngeal RT-PCR assay. We advocate the use of stool-RT PCR for COVID-19 to confirm the pathological role of COVID-19 on acute appendicitis [Table 1].[1],[2]{Table 1}
